Adoption Application

Pup Squad Animal Rescue works hard to find the perfect home for all the dogs and cats in our care.

Our adoption process is as follows:

  1. All applicants must first complete an Adoption Application. You can either complete the application and submit it using the online form below (compatible with desktop browsers only- no mobile devices or tablets) or download the form in either Microsoft Word or Adobe Acrobat. Save it to your computer, fill it out, and then email it to adoption@pupsquad.org.
  2. After receiving your application, we will contact you to discuss your current situation and which pet you are interested in adopting.
  3. After discussion, we will determine if we think this could be a potential good match. If so, we can arrange a time for you to meet your prospective new family member.
  4. Upon agreement by all parties, sign adoption contract and pay the adoption fee for your newest family member. The adoption fee is a $200 donation to Pup Squad. Pup Squad accepts cash and credit/debit cards for adoption fees. Credit/debit card transations will incude a $5.00 fee to cover the processing charges. This includes the first two rounds of vaccinations as well as the rabies vaccination, spay/neuter and microchip of each animal. Please remember all breed estimates are just that...estimations based on our experience. We can make no guarantees as to the breed mix.
